Please find below our new patient forms for Core Chiropractic & Methodist Physical Therapy. Please take a moment to print these and fill them out. Please note that there maybe more than one link to click for all of your patient forms. Please then bring these with you to your appointment. This paperwork provides us with general information about your medical history and any current problems you are experiencing.

Dr. Brown & Dr. Rogers, your Flower Mound Chiropractor & Physical Therapist will review all of your paperwork with you. They will talk to you about your medical history, and will ask various questions related to your condition. Finally, if necessary, they will perform specialized clinical testing. This might include labs or imaging, in order to make a specific diagnosis of your condition.

If you have any issues downloading our paperwork please do not worry. We will have a copy in office for you to fill out. Please just come in 10-15min early to allow time to fill these out.
