This paperwork provides us with general information about your medical history and any current problems you are experiencing.
Dr. Brown, your Flower Mound Chiropractor, will review all of your paperwork with you. He will talk to you about your medical history, and will ask various questions related to your condition. Finally, if necessary, he will perform specialized clinical testing. This might include labs or imaging, in order to make a specific diagnosis of your condition.
If you have any issues downloading our paperwork please do not worry. We will have a copy in office for you to fill out. Please just come in 10-15min early to allow time to fill these out.
